CDC Moratorium Declaration

This declaration is for tenants, lessees or residents of residential properties who are covered by the CDC’s order temporarily halting residential evictions to prevent the further spread of COVID-19.

Under the CDC’s order, you must provide a copy of this declaration to your landlord, owner of the residential property where you live, or other person who has a right to have you evicted or removed from where you live.

Application FAQs — Arkansas COVID-19 Fresh Start Housing Stabilization Program

1. The first step in the application process is to go to the map at the bottom of this page, here.
2. On the map, click on the county that you live in. This will highlight your local Community Action Agency. (All counties in the agency’s service area will be highlighted, including yours.)
3. To the left of the map, click “Apply.”  (If the “Apply” button does not appear, it is because the agency that serves your county and area is not currently accepting new applications.)
4. You will be taken to a page that says “Accept Invitation.” Here, you will enter your email address in the email field and click “Accept.”
5. You will be taken to a new page that asks you to enter some basic information.
6. After you have submitted this information, you will be placed on a waiting list.  To be moved from the waitlist, the agency will have to assign your application to a case manager. The timeframe will depend on the agency’s capacity to process applications already in their queue.
7. Once a case manager is assigned your application, you will receive an email from no-reply@elogicgenesis.com ​inviting you to begin your online application. Be sure to keep an eye on your “spam,” “junk,” or “clutter” email folder, in case the email goes there.

It is important to note that the funds for the Arkansas COVID-19 Fresh Start Housing Stabilization Program are first come first serve, and it is possible that not every eligible applicant will receive assistance if funds are exhausted.

8. After you receive the email inviting you to begin your online application, go here for additional instructions.

You will have ten (10) calendar days to complete your application once you’ve begun. Incomplete applications after ten days will be denied and you will need to reapply.

If you were denied benefits because your landlord refused to accept payment or if you are currently homeless or staying with family or friends due to having no other housing options, you can reapply for benefits.

If you qualify, the Fresh Start Program will pay an amount up to one month’s rent as necessary to pay a security deposit at a new dwelling, and 1.5 months of rent for the new dwelling. The new landlord must agree to accept payment from the program.

If you need to reapply in the future, log into your account and repeat the application process – DO NOT create a new record using the link from the AR Fresh Start website.

For the purposes of the Arkansas Fresh Start COVID-19 Housing Stabilization Program, applicants who have “rent-to-own” and “lease-to-own” agreements are qualified to apply for the program as these types of agreements are considered similar to rental agreements. You will need to be able to give documentation related to these agreements to your caseworker.

If you clicked the wrong link in the AR Fresh Start Website, DO NOT enter your email address. Return to the website and click the correct link.

If you have already entered your email address and created your account, follow the instructions shown here, then return to this article when finished to continue the application process.

After you click the link from the Arkansas website, you will be brought to a new page that reads “Accept Invitation”.

Enter your primary email address, click Accept, then stay on this page!

You will receive an email with the subject “eLogic Genesis Email Confirmation Requested” from no-reply@elogicmodel.complease wait up ten minutes for the email to be delivered. If you do not see it, check your spam folder and add no-reply@elogicmodel.com to your address book.

When you receive the email, open it and:

1. Enter the four digit Confirmation Code from the email into the “Accept Invitation” page under Email Confirmation Code
2. Create a password for your account by entering it in the Password and Confirm password fields
3. Enter your First Name and Last Name in the text fields
4. Select AR from the State drop-down list
5. Locate and select the county you live in from the County drop-down list
6. Click AcceptIf you receive a message that reads “You already have a record in this agency”, contact the agency and speak with a caseworker

1. On the left side of the screen, click Apply for Programs
2. Click the checkbox next to “The Arkansas COVID-19 Fresh Start Housing Stabilization Program”
3. Click Save
4. Continue to “Enter Your Information” below

ENTER YOUR INFORMATION

You will need to enter your and all adults’ (age 18 or older) information in each intake tab.
1. The “Intake” tab is your information
2. “Household Member 2” = second adult’s information
3. “HHM3” = third adult’s information
4. “HHM4” = fourth adult’s information
5. “HHM5” = fifth adult’s information

DESKTOP
1. Click the Intake tab from the navigation bar
2. Enter your information – fields with a red outline are required
3. The system will save as you go
4. When finished, scroll to the bottom of the page and click Next – you will be moved to the Household Member 2 tab
     • Enter the second adult’s information
     • If you are the only adult in the household, skip to Step 6 below
5. Continue entering all adults’ information under each HHM tab
6. When all adults’ information has been entered, click the Forms tab
7. Continue to next FAQ, “How do I fill out and submit CDC Moratorium Declaration form?”

MOBILE
1. Be sure “Intake +” is shown in the menu at the bottom of the screen – if it is not, tap the menu, then tab Intake

2. Enter your information – fields with a red outline are required

1. On the Forms page, locate the CDC Moratorium Declaration form and click Create
2. Read the form
3. Know that by entering your name in the box that you are agreeing to, under penalty of law, all of the stipulations contained in the CDC Eviction Moratorium Declaration
4. Type your name in the Signature text field
5. Click Finalize
6. Click the Budget tab from the navigation bar -OR- Scroll up and click <— All forms, then click Next
7. Continue to the next FAQ, “How do I enter household income information?”

MOBILE
1. On the Forms page, locate the Landlord Form and click Create
2. Read the form
3. Know that by entering your name in the box that you are agreeing to, under penalty of law, all of the stipulations contained in the CDC Eviction Moratorium Declaration
4. Tap the Signature text field and type your name
5. Click Finalize
6. Tap the navigation menu at the bottom of the screen (where it says Forms +)
7. Tap Budget
8. Continue to the next FAQ, “How do I enter household income information?”

You will need to enter your and all 18+ household members’ income.

All income entered must also have proof of income documentation uploaded your record for the previous month (if applying in November, provide October’s proof of income documents).

!!! If any household members over the age of 18 have income, it must also be entered. You can enter up to four other household members’ income in the boxes below following the same steps addressed below under Desktop or Mobile:

1. On the Budget page, click the “Income – 1st Household Member” box to expand to enter your income:
     • Locate the Income Type
     • Click $ Add income source
     • Enter the Amount
     • Under Frequency, click the drop-down list and select Daily, Weekly, Bi-Weekly, Monthly, Yearly
2. Repeat for each of your sources of income
3. Scroll to the top of the page
4. Click Save changes (located above the blue box on the right)
5. When finished:
     • Click the Files tab from the navigation bar
     • Continue to the next FAQ, “How do I upload files?”

1. Scroll down on the Budget page
2. Under Income Options, tap the “*Your Income Sources” box to expand:
     • Locate the Income Type and tap to expand

In order to submit your application, you will need to upload the documentation listed below and label each file.

Your caseworker may contact you and send you other files to fill out and upload.

One form of ID
     • On the intake page, you selected a “Type of Identification Provided (One form of ID required for applicant only):” drop-down list. Be sure you upload the form of ID you have selected.
First page of Lease/Tenant Agreement
     • Your caseworker will need either the first page of your Lease or Tenant Agreement that shows the important information about your rental. If you do not have a lease or tenant agreement, a rent receipt will work.

Proof of income documentation for each entered Income Type for each person
     • Remember that you must submit the previous month’s income. For example, If you are applying in November, submit October’s documentation. If you are applying in December, submit November’s documentation.
     • If your household has no income sources, please refer to the FAQ, “What if I have no sources of income?”  Your caseworker will provide an additional document for you to fill out and upload

3. Locate the file(s) from your computer
     • You can upload up to three files at a time
4. Select the file(s) and click Open
5. Under the Req. Files (Application) column, click the + plus sign

You will receive an email and system notification that your application has been submitted and is under review. Your caseworker will begin processing your application after it is submitted and will be in touch with next steps.
